When you make your first appointment, you will be told to come in early to fill out information forms. If that is not convenient, we encourage you to print out the following forms and fill them in at your leisure. Bring them with you when you come in for your appointment so that they will be kept in your permanent record.
Insurance Plan Information
Today’s health insurance policies and coverage offer more options (and confusion) than ever. Each patient is responsible for knowing his or her plan’s benefits package, co-payment, deductible, non-covered services, and restrictions. Many plans have participating or preferred providers. We participate in most area health plans. The following list are some of the plans that we accept.

- Blue Care Net work
- Blue Cross Blue Shield of Michigan (All plans)
- Care Choices
- DMC Care
- United Health Care
We are in the application process for other plans. Check with us if your plan is not listed above.
Out of plan patients:
Internal Medicine and Primary Care Specialists can continue to be your doctors if your insurance plan accepts “out-of-network” providers. Most plans have coverage for patients who see doctors not in the network or participating with the plan. This is called “out of network” benefits. Patients who have out of network benefits may have to pay deductibles and co-pays for services by a non-participating provider. Patients should consult with their plan about their out of network benefits. If you have questions regarding the benefits of your plan, we have a knowledgeable billing department that may be able to help you. We ask that you share both your concerns and knowledge of your insurance benefits with us.
